If you're on the PC, there are mods that allow you to start creating a character from a preset that has those bigger ears, though I've not created a character using one so don't know if it causes any issues. The models available in the default character creator don't include the sticky-out ears.

Maybe it's because it's a Female Elf PC i'm trying to make? I don't think that's the case because i've seen some female NPC elves with big ears. It could be done, like the DLC items, Not the place I know, but it was on topic. Oh yes, and I would like to whine about the fact that console players cant upload characters using the character creator. Does seem a shame that even Dalish Elves are forced to be flat ears due to a racist lack of ear holes in most helmets.
